This is a reminder that all students entering pre-school, kindergarten,3rd grade, 6th grade, and new students must turn in a physical by August 12 to the school nurse. The physical can be from the 12 month period before school starts. Those students entering kindergarten must have received their 5 year shots in order to be in school. Any medications that are going to be given at school, including over the counter meds, must be sent in their original container and have a signed permission form from your pediatrician. If your child has a food allergy or asthma and requires an Epi-pen or inhaler they will require an action plan for both.  Please email Mrs. Zimmerman if you have any questions at mazimmerman@mqpwg.org
